The Coordinated Community Care (CCC) Social Worker Program Coordinator (SWPC) serves as the program manager for the Contract Nursing Home Program (CNH), Home Based Primary Care (HBPC) and Purchased Community Care Programs including but not limited to: Contract Adult Day Health Care (CADHC), Homemaker/Home Health (HHA), Hospice and Visiting Nurse. The incumbent also serves as the State Veterans Home Liaison. BASIC REQUIREMENTS Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree
