The HUD-VASH LPN provides supportive services and complex care for Veterans enrolled in the VA homeless program. The HUD-VASH program provides a Housing First model of care, and the Veteran population served may experience chronic/recurrent homelessness, active substance abuse, have complex medical needs and/or mental illness. The LPN helps Veterans in the program with housing in their community of choice, via the provision of age appropriate medical and psychiatric assistive care.
