This Social Worker is assigned to Lebanon Primary Care, Patient Aligned Care Team (PACT), Specialty and Spinal Cord Injury Clinics with responsibility to provide social work services to adult and geriatric Veterans. Provides open access and immediate response to Veterans in psychosocial crisis in the Lebanon Primary Care and Specialty Clinics to include supporting their behavioral health needs. Primary Care coverage includes services to all Veterans without regard to era of service. BASIC REQUIREMENTS. The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. § 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following: Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part).Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). GRADE DETERMINATIONS. In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.(c) Ability to supervise multidisciplinary staff assigned to the program.(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).(e) Ability to provide training, orientation, and guidance within clinical practice. References: VA Handbook 5005/120 Part II Appendix G39 ["VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The Patient Aligned Care Team (PACT) is a comprehensive team which delivers primary care to Veteran patients in a longitudinal rather than episodic fashion and which has as its focus: prevention; health promotion, coordination and chronic disease management. The social worker in this Veteran-centric approach often functions as a care coordinator with a panel of Veterans to ensure that health care meets the needs, as defined by the Veteran. This professional Social Worker is responsible to provide a variety of services for Veterans assigned to a Lebanon Primary Care Provider, or on a walk-in basis including psychosocial assessments, treatment planning, short-term, and solution focused counseling with individuals and families who may be in crisis, education and referral for resources and benefits, and case management. Responsibilities will also include consultation and education to Medical Center staff and service providers, the collection and maintenance of statistical data, program planning evaluation and modification, and the supervision of students as assigned. Collaborates with the other members of the Lebanon and CBOC Primary Care Clinics in the provision of comprehensive health care services to Veterans, ensures equity of access, service, and benefits to this population, and collects, develops and analyzes data from this program, using appropriate clinical indicators and monitoring activities to ensure that the care provided is of the highest quality. Completes periodic program planning evaluations and reviews, and recommends and implements new programs and/or modifications/expansion of existing programs, as indicated. Responsible for the implementation of written treatment program policies and procedures developed by Social Work Service that are based on coherent empirical and theoretical rationale and are consistent with patient rights and the maintenance of comprehensive written patient progress notes and outcomes. Other duties and responsibilities may be assigned by the team/group leader as needed for the benefit of the Veteran, family, and quality of services provided. The individual works in Lebanon Primary Care Clinics but may be required to work an irregular tour of duty, weekends, depending on the needs of the service and participate in social work coverage to the medical center between 4:30 p.m. and 8:00 a.m. The individual performs administrative and program-coordinating functions within several diverse Primary Care and Specialty Clinics characterized by multiple objective (treatment, education, training and research), as needed. The incumbent also serves as the Spinal Cord Injury (SCI) Social Worker.
