The VA Northeast Ohio Healthcare System is recruiting for a Nurse Practitioner in Primary Care Service at the Carl Nunziato VA Clinic. This is an OPEN CONTINOUS ANNOUNCEMENT and will remain open until Thursday, June 30, 2022 or the position is filled. The initial cut-off date for referral of eligible applications will be Friday, April 22. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis. In order to qualify for this position you must meet the following requirements: Citizen of the United States. Graduate of a school of professional nursing approved by a State accrediting agency and by one of the following accrediting bodies at the time the program was completed: The National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted registration/licensure as a nurse practitioner in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. A Master's Degree from an academic program accredited by the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE). A full and current certification as a Nurse Practitioner from the American Nurses Association or another nationally recognized certifying body. Preferred Qualifications: Adult/Geriatric/Acute Care Nurse Practitioner Master's or completion of ACNP/CCNP post Master program strongly preferred. At least two year's experience treating the chronically ill in a primary care practice. Experience managing a complex patient cohort within an ambulatory care setting. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree. Physical Requirements: This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Will provide comprehensive primary care services to a defined panel of patients. Duties include history taking, physical examination, ordering laboratories and diagnostic studies, diagnosis and formulation of treatment plans for acute and chronic conditions within the scope of Medicine practice. This provider is the leader of a Patient Aligned Care Team (PACT) (Nurse Practitioner, RN, LPN, and clerical associate) which provides direct care for Veterans enrolled in Primary Care at the Carl Nunziato VA Clinic. This provider is assigned responsibility for a panel of patients at a specific location. However the provider may be asked on some occasions to assist in patient care or administrative duties at the main hospital or at another site. Providing clinical examination, diagnosis, and treatment of acute and chronic diseases, preventive care, and coordination of care for a panel of primary care patients. Appropriate documentation of patient care in clinical records is expected. The Nurse Practitioner refers to specialists when additional input and treatment outside of the scope of the Nurse Practitioner's experience and training is required. Responsible for monitoring PACT team performance to assess adherence to VA standards, assure quality and effectiveness of care, and meet VA performance measures. Outstanding customer service is expected. Leading the team in assisting the patient advocate and administration in resolving patient concerns and inquiries. Exercising medical decision-making and providing a broad range of diagnostic and therapeutic services for a specific group of patients; Must have excellent communication skills and relations with staff and community and is able to elicit cooperation from others. The primary care physician serves as a member of a larger multidisciplinary treatment team which includes social work, pharmacy, mental health, nutrition, and on occasion, other services. Work Schedule: Monday through Friday from 8:00 a.m. to 4:30 p.m. EDRP: This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.
