The PTSD Admissions Coordinator, Social Worker is assigned the PTSD Residential Rehabilitation Treatment Program (PRRTP) with significant responsibility for coordinating the referrals to the PTSD RRTP and provide direct services for veterans who are admitted for care, with particular emphasis on patients participating in the PTSD Residential Rehabilitation Treatment Program(RRTP). Coordinate the admissions process with veterans, referral sources, and PTSD RRTP interdisciplinary treatment team. This includes the day to day management and operational aspects for veterans who request admission into PRRTP. Serve as a point of contact for all inquiries (within the VA system and outside the VA system) regarding admissions to the men's and women's PTSD RRTP. Complete psychosocial assessments upon admission to determine veteran's needs while in the program, as well as his/ her recovery goals. Is an active participant in the treatment planning process with other disciplines. Participate in discharge planning with other disciplines and is responsible for ensuring that discharge plans are executed in a matter that is timely and appropriate. Provides evidenced based treatments via individual and or group therapy, as appropriate. Assists interdisciplinary team with referrals to a variety of appropriate programs upon discharge from the program, such as MHICM, Psychosocial Rehab and Recovery Program, and facilitates appointments for aftercare. Provides individual therapy to veterans, as needed, throughout the continuum of care. Addresses Advance Directives and Organ Donation in accordance with the standards of their work area. As appropriate, supervises the practice of social work interns, or a less experienced social worker. Participate in committee assignments, as deemed necessary by their supervisor. Maintain data and statistical compilations to comply with Joint Commissions, VA, and medical center policy and procedures, Function as a liaison with various drug courts and veteran courts. Assure accurate and timely documentation, using appropriate notes and templates as defined within the facility PTSD RRTP unit and behavioral health. Other duties and responsibilities may be assigned by the supervisor as needed for the benefit of the veteran., family, and quality of services provided.
