This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Community Care Social Worker is responsible for oversight of non-VA episodes of care and Social Work Coordination between VHA, VA Community Care, the Veteran and Family, and community providers. This Social Worker will function independently within their scope of practice. Major duties include, but are not limited to the following: Independently works with veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems. Serves as the Social Work Subject Matter expert to Community Care interdisciplinary team. Primary duties will include but not be limited to assisting all Community Care interdisciplinary team members with: Consult management; tracking and movement of Veterans between care facilities and VHA, and assisting in the resolution of stalled consults due to complex psycho-social needs. Frequently offers consultation to colleagues and students on the psychosocial treatment and care coordination of complex patients as incumbent is seen as an expert and leader among peers in the psychosocial care of medically complex patients. Possesses a working knowledge of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, substance abuse disorders, bereavement, common medications, and general medical terminology. Has the ability to quickly learn about unfamiliar diagnoses in their effort to coordinate care. Possesses a high level of skill and expertise to establish and maintain effective therapeutic relationships with, and in assessing and treating the complicated psychosocial problems of, veterans and their families/significant others.
