The incumbent is a Licensed Professional Mental Health Counselor within the Jesse Brown VA Medical Center's Mental Health Service Line (MHSL). Major Duties: - Conducts structured interviews and screenings to assist in the assessment of patients with substance abuse problems and related psychosocial issues with the intent of determining appropriate interventions and recovery needs. - Ensures the initiation, completion, and documentation of a psychosocial assessment on new patients and the development of preliminary treatment goals. - Provides individual or group counseling on addiction-based behaviors and substance abuse issues, including interventions focused on enhancing abstinence, moderation and relapse prevention. - Manages and coordinates major aspects of mental health care for an assigned caseload of patients including the development, review, update or modification of individualized treatment plans. - Provides evidence-based psychotherapeutic interventions for substance abuse, including individual, family, and group psychotherapy. - Provides skills building and educational groups to patients and families, working with families and significant others to assist in recovery initiatives, continuity of care, community integration, recovery and relapse prevention. - Assists in transitioning patients to appropriate levels of care and ensures continuity of care across clinical sections, divisions, facilities, and organizations. - Makes telephone and written inquires to assist patients and enhance care coordination. - Assumes complete professional responsibility for his/her clinical assessment findings, patient care decisions, and documentation. - Applies age-based and cultural diversity knowledge to individualized services provided to patients.
