The incumbent functions as the Case Manager for the Health Care for Homeless Veterans (HCHV) Program, within the Social Work Service Line. The HCVC Liaison is responsible for monitoring and coordinating services provided by community-based programs funded under VA's Homeless Providers Contract Housing Program. The HCHV Liaison is also the key point of contact between these community-based providers and the G.V. (Sonny) Montgomery VA Medical Center. Duties performed include, but are not limited to the following: Works in coordination with the HCHV Program team to provide a seamless system of healthcare services under the supervision of the HCHV Program Coordinator; Serve as the Contracting Officer Technical Representative (COTR) and as such will make recommendations about contract renewal to the VISN (Veterans Integrated Service Network) Contracting Office; Provides administrative oversight of the HCHV contract funded programs as defined in the HCHV rules, regulations, and directives; Coordinates and participates in annual program site inspections utilizing the HCHV Program rules, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices; The HCHV Liaison serves as the Point of Contact for the National Call Center for Homeless Veterans and follows up with area veterans by providing access to emergency housing, assessment and VAMC or community services/resources; Directly coordinates admission to contracted HCHV emergency hosing providers; Provides direct services to veterans such as conducting psychosocial assessments; collaborating with veteran to develop an initial plan of care; providing advanced level case management; providing crisis intervention services; assisting veterans with accessing resources to enable them to work toward safe, affordable and quality permanent housing; Provides case management at homeless community-based program sites; Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies; Engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless veterans as well as provide current information on services and benefits available in the VA for Homeless Program participants; Participates in the orientation/teaching of graduate social work/nursing students, students of other disciplines, and KCVA personnel on the biopsychosocial aspects of homelessness.
