How would you like to become a part of a team providing compassionate care to Veterans? The Senior Social Worker is assigned to the MH Integration team. Must use a high level of skill in assessing and treating the psychosocial problems of Veterans and their families/significant others. The Primary Care Mental Health Integration Social Worker provides Behavioral Health services and interventions to the Veteran population enrolled in Primary Care. Services/interventions include diagnosing mental disorders, conducting brief psychological assessments, treating mental disorders through brief modalities and providing adjunctive interventions for the treatment of medical disorders. Among the modalities provided are individual, couples, and group psychotherapies; this includes short-term, solution-focused behavioral health interventions and evidence-based interventions to address behavioral health needs to improve overall wellness and quality of life as well as provide crisis intervention services. The PCMHI Social Worker is familiar with resources/policies of the community they serve and often function separately from the main hospital. The PCMHI Social Worker will provide ongoing consultation with the Veteran's Primary Care Provider to ensure collaborative care of the Veteran's treatment needs. Independently completes thorough assessments to determine the underlying causes of the presenting problem(s), the interpersonal and environmental factors impacting the problem, and its effect on the patient's ( ability and desire to comply with the treatment recommendations by the multidisciplinary team. Facilitates connecting Veterans with the most appropriate behavioral health services. Uses a population-based approach (1-6 sessions) to address behavioral health concerns for Veterans in need of only short-term care. Provide telephone and/or telehealth PCMHI services to other community based clinics of the Edward Hines, Jr. VA. Participates in first point of contact assessment and coordination of mental health needs for walk-in patients in crisis situations or enrolling in VA care at that CBOC. Responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the Veterans Integrated Service Network (VISN), and the facility. Responsible for furthering one's own professional growth through continuing education and for ensuring continuing education appropriate to the area of assignment. provide assistance with the orientation and learning of less experienced social workers and, if interested, to serve as a preceptor for social work students. The incumbent is under the direct supervision of the Social Work Clinical Manager through the Chief of Social Work Services and receives clinical, administrative guidance as it relates to Social Work practice. The PCMHI Team Leader will collaboratively provide administrative and clinical supervision. The incumbent uses word processing software to execute several office automation functions such as storing and retrieving electronic documents and files; activating printers; inserting and deleting text, formatting letters, reports, and memoranda; and transmitting and receiving e-mail.
