This position is located in the in the Nursing Mental Health unit at the Nashville VA in Nashville, TN. RN VA Pay Scale NG 57 Mental Health Intensive Case Management (MHICM) is a community based program for veterans with very disabling mental health problems who live within an hour's drive of the Nashville Veteran's Administration Hospital. The program provides mental health services to eligible veterans in their homes and communities. The goal of the MHICM program is to support veterans in their recovery goals and minimize demoralizing crises. To do this the MHICM team members provide: The RN's role will include, but is not limited to the following duties: Frequent home or community visits. Instruction, education and counseling about severe psychiatric conditions. Support around developing coping skills. Assistance with addressing housing, income, vocational and/or family concerns. Referral to community resources and services. Care Coordination of other VA services and community services according to veteran's needs. Education and support to family and close friends. Administering and evaluating pharmacological and other therapeutic treatment regimens. Administering medicine Establishing priorities and making ethically-sound decisions to ensure safe patient care. Responding to emergency situations to promote optimal patient outcomes by recognizing changes in patients' health status. Documenting patient data to ensure continuity in the provision and coordination of patient care. Collaborating with other health care professionals. Communicating with veterans and family members regarding veteran's care. Acting as resource to others. Participating in continuing educational achieving/maintaining certification.
