This Social Worker position provides comprehensive assessments and subsequent updates on all Veterans coming into the Domiciliary Health Maintenance Program (HMP). The HMP Veterans are more complex in terms of serious mental illness as well as co-occurring disorders, higher numbers of co-morbidities, requiring an ever more dynamic clinician that provides more frequent encounters with Veterans. Responsible for completing high-risk assessments on Veterans in the program who are reporting suicidal or homicidal thoughts, and for ensuring safety planning following completion of the high-risk assessment. Provides ongoing individual case management, and facilitates groups and classes as needed. Crucial discharge planning for Veterans successfully completing the program and who are re-entering the community may also be part of the treatment provided. Utilize assessments in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable. Working knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic, and traumatic illnesses, common medications and their effects/side effects, and medical terminology. Possess the knowledge and ability to independently implement treatment modalities, provide individual or group counseling and/or supportive groups for Veterans and their families. Maintain knowledge of Veteran benefits and services, community resources, and process for making appropriate referrals to community and other governmental programs or agencies. Communicate effectively, both orally and in writing, with people from varied backgrounds. Willing to serve on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Chief of Mental Health, Domiciliary or Social Work Services. Responsible for furthering one's own professional growth through continuation and for ensuring continuing education appropriate to the area of assignment. Provide assistance with the orientation and learning of less experienced social workers and, if interested, to serve as a preceptor for social work students.
