The VA Central Iowa Health Care System in Des Moines, IA is currently recruiting for one (1) full-time Licensed Practical Nurse to work on the Community Living Center CLC-2 Unit with the Extended Care and Rehabilitation Service Line. The LPN is responsible for providing care to a veteran population with widely diverse medical, skilled, mental health and end-of-life needs. The incumbent is responsible for direct patient care activities under the supervision of the Registered Nurse. The LPN is responsible for goals and interventions that promote veterans in attaining maximum functional potential. This is accomplished through but not limited to modalities such as ambulation, bowel and bladder training, dressing, grooming, bathing, oral hygiene, range of motion exercises and fall prevention modalities. Duties include, but are not limited to the following: Demonstrates excellent customer service skills in communicating with the patient, families, co-workers and interdisciplinary team members. Observes, identifies, and responds to patient's need for care, including medication, equipment-assisted care, patient/family education, and oral/written documentation in accordance with policy, procedures, and guidelines. Prepares/administers prescribed medication (oral, topical, subcutaneous, and intramuscular), following standard medical center policies. Adheres to the agency requirement for reporting medication errors, reactions, and side effects immediately. Accurately documents patient information on flow sheets and charts in electronic format. Participates in the interdisciplinary team care planning and delivery of care process. Implements orders and provides care to variable patient population. Recognizes emergency situations and demonstrates effective action in emergencies. Provides direction to nursing assistants in providing nursing care and activities of daily living (ADL) cares for resident Veterans. Collaborates with multidisciplinary team to provide interventions tailored to each resident's needs to improve engagement, optimize function and decrease distress. Assists with various unit activities including pleasant events, restorative therapy and recreation activities to promote a stimulating and engaging environment. Preferred Experience: Flexibility in schedule and assignments for a 24/7 facility Highly motivated, self-directed individual with strong work ethics 1-2 years of LPN experience Poised and articulate in communicating with individuals of varied education and ethnic backgrounds Rehab/Restorative Care Experience Collaborative and effective team member Organizational and time-management skills Work Schedule: Full-time Nights, may include 8 hour and 12 hour shifts including weekends and holidays (11:30pm-8:00am, 7:30pm- 8:00am) Financial Disclosure Report: Not required
