The Mental Health Intensive Case Management (MHICM) social worker is responsible for providing intensive case management services to enrolled Veterans, primarily in the community. Services are provided within a 60-mile radius of Central Arkansas Veterans Healthcare System in North Little Rock, AR. Veterans enrolled must be seen at least once a week and as often as clinically indicated. The incumbent assists participants' families and clinical team members in coordinating care in the community and resource, when clinically indicated, and works in unison with the Interdisciplinary Treatment teams in promoting a Patient-Centered culture; assuring that quality health-care services are provided to the Veterans promptly, safely, courteously and compassionately as well as their significant others and/or major stakeholders are appropriately involved. The incumbent provides patient, family, and/or caregiver education and support, as well as actively promotes patient self-management and personal health responsibility. The incumbent partners with the Veteran t develop treatment/recovery plans and works closely with the Veteran to reach his/her goals The incumbent conducts comprehensive psychosocial assessments, suicide prevention screenings, and safety plans. The incumbent communicates effectively both orally and in writing with professional, administrative and technical staff at all levels. The incumbent functions as liaison between internal and external resources, including community agencies, courts, probation and parole officers, guardians, conservators and providers concerning appropriate notifications. The incumbent collaborates and shares information with medical and/r clinical treatment staff members regarding potential improvement in patient care delivery through comparisons to community standards. The incumbent demonstrates effective communication skills and behavior that promotes professional cooperation and team building with internal and external customers. The incumbent performs screenings to assist in determining Veteran's eligibility status for MHICM enrollment The incumbent shares information t assure continuity and safety in area f assignment and collaborates with co-workers in completing workload. The incumbent participates on teams, contributes to team conferences and is actively involved in planning for continuity of Veteran care. The incumbent consults with other healthcare providers to formulate a collaborative plan of care. The incumbent performs other duties as assigned.
