The VISN 10 Clinical Resource Hub is recruiting for a Physical Therapist. The Physical Therapist will function virtually within the VISN 10 Clinical Resource Hub (CRH), a repository of clinical and administrative staff that serve VA facilities within VISN 10 that are underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Conducts comprehensive patient examinations to identify rehabilitation problems and establish treatment goals utilizing the most recent evidence based Physical Therapy practice. Develops and implements treatment plans designed to meet established goals. Evaluates patient response to treatment and modifies the treatment plan to provide optimum outcome. Provides education and counseling of patients and families. Conducts examinations, treatment planning and interventions, documentation, and follow-up evaluations within the outpatient setting, via telehealth modalities, but may also support wellness programming, inpatient, HBPC, CLC, and other settings. Demonstrates competence in the examination and treatment of patients in the following areas, but is not limited to: Geriatric Rehabilitation, Musculoskeletal Disorders, Cardiopulmonary Rehabilitation, Stroke and Head Injury, Arthritis, Spinal Cord Injury, Chronic Pain. Amputation, Vestibular Rehabilitation, Post-Surgical/Joint Replacement, Assistive Technology, and Health Promotion Demonstrates competence in the appropriate selection of functional outcome measures as related to patient management. Conducts record keeping related to patient care documentation and quality improvement (QI) data collection as specified by CRH leadership. Oversees appointment scheduling for assigned patients as needed including travel arrangements in coordination with CRH support staff and per Advanced Clinic Access (ACA) guidelines. Responsible for the examination and treatment of patients referred to CRH PT services and telehealth initiatives. Provides in-service and clinical training programs as assigned by CRH leadership. Designs, conducts, and evaluates education experiences for staff members, and seeks educational opportunities to enhance the PT services throughout the VISN and under the CRH. Establishes and maintains contact with other key staff involved in patient care via written, telephone, and personal communication on a regular basis at the VISN, spoke sites. Develops and provides timely reports to CRH leadership. Maintains timely data input utilizing care plans, initial assessments and/or specialty protocols as part of an ongoing process in coordination with other key interdisciplinary staff to provide effective patient care. Represents the CRH PT at various clinical and administrative meetings/rounds at the discretion of CRH leadership. Serves as a teaching Physical Therapist for students as assigned. Performs other tasks as assigned by CRH leadership that are consistent with the mission of the VISN 10 CRH. Work Schedule: Monday-Friday. 8:00 am - 4:30 pm or 07:30am - 04:00pm depending on spoke site needs; evenings, weekends, and holidays required as necessary based on the needs of the CRH / spoke sites.
