The Registered Nurse assigned to the Community Living Center (floor 2 and 3) is a professional nurse with a broad-based clinical knowledge that enables him/her to effectively provide skilled care to a veteran population with widely diverse medical, mental health and end-of-life diagnoses. - Accountable for all elements of the nursing process - Assesses, plans, implements, and evaluates care based on age specific components - Responsible for coordination of care focused on the veteran's transition through the continuum of care, including transition through end-of-life - Responsible for veteran and family education - Responsible for veteran self-management after discharge - Responsible for supporting factors that impact customer satisfaction - Considers all characteristics of the individual including age, life stage, state of health, race and culture, values, and previous experiences to provide optimum veteran centered care. - Administers medications and procedures per established policies and guidelines - Collaborates with interdisciplinary team members to positively influence veteran care outcomes - Work with team members supporting veterans and family members utilizing a holistic approach through end-of-life - Other duties as assigned Work Schedule: Various schedules are available.
