The Healthcare for Homeless Services (HCHV) Social Work provides outreach, supportive interventions, and case management services to Veterans experiencing or at imminent risk of homelessness, who may have mental disorders and/or substance abuse disorders, at Harry S. Truman Memorial Veterans Hospital and local emergency shelters.. The HCHV Social Worker is responsible to assisting Veterans and their families in moving out of homelessness and/or chronic homelessness.
