This position involves working with individuals with severe and persistent mental illness. Roles and responsibilities include but not limited to completing community assessment to assess goals, ensure consumer receive timely continuity of care, assisting with individual service coordination and linkages, consumer advocacy, training in daily living skills, direct assistance with structuring and performing ADL, consumer psychoeducation and support services, crisis intervention services, medication education and health care monitoring and oversight services.
Bachelor's Degree in helping profession and 1 year of experience working with adults in the mental health field. Valid NJ Driver's License required without any restrictions. CPR Certification, computer literate, Crisis Intervention Training, organizational ability, clinical skills, excellent communication and interpersonal skills are required. Bilingual (Spanish or ASL) preferred, but not required.
