Reporting to the Support Services Manager, Hospice and Homecare, you will assist and counsel patients and families with financial, social and emotional concerns, and utilize your effective interpersonal skills to diffuse complex, crisis patient and family situations.
Masters of Social Work (MSW)
Current License in the state of Pennsylvania (LSW) preferred, or ability to obtain within 1 year of employment
2+ years of social work experience in a health/mental health related field
Ability to travel to patient care assignments over a multi-county area
Current driver’s license, and personal automobile insurance
