Description The goal of the Behavioral Health (BH) Clinical Navigator Team is to capture every BH patient that comes through the Emergency Department for effective evaluation and intervention. Responsible for managing, coordinating and integrating all Care Coordination services. Creates a Behavioral Health Triage and Referral Model that navigates BH patients to pathways within the hospital and/or within the wider community for supportive services. The Clinical Navigator will train and supervise the Peer Support Workers in the Emergency Department to utilize the Triage and Referral Model to make linkages within the hospital and the wider community . Provides Care Coordination services to patients based on person-centered acuity, and leads and coordinates patient care coordination interdisciplinary team in a clinical setting. Contributes to a cooperative and accountable working relationship with other members of the Care Coordination system staff toward the goal of providing continuous services. Promotes optimal person-centered care that supports and empowers individuals, respects individual choices, and meets health care needs of patients. BH Clinical Navigator may provide intensive clinical case management by maintaining a small client caseload as needed. Requirements EDUCATION: Masters in Counseling, or Masters in Social Work, or BSN in Nursing (optional)
CERTIFICATION/LICENSES: New Mexico LPCC, New Mexico LCSW, or LMHC, LMSW with attainment of full associated NM licensure within 6 months. Current New Mexico Nursing License if RN.
SKILLS: Knowledge working with diverse populations struggling with substance abuse and behavioral health issues. Considerable knowledge of health care facilities, policies and issues, and of the full spectrum of community patient care services. Considerable knowledge of health care and social management principles, practices and procedures. Commands a full understanding of BH issues and barriers to care, with intensive clinical case management of high acuity populations Ability to coordinate various functions and activities for maximum cooperation and integration of services providers and persons receiving care within a variety of clinics for continuum of care services. Ability to direct, instruct and advise staff, and to receive and effectively react to day-to-day problems. Ability to utilize strong communication skills, both written and oral, and effectively demonstrate an interactive style of care coordination. Ability to understand and implement sensitivity and culture of care measures appropriate to a diverse population. Ability to work with various levels of the organization including clinical, financial, administrative and medical staff. Ability to coordinate multiple departments, as well as community agencies, to reach collaborative goals. EXPERIENCE: Minimum of 2 years of experience in a health care setting preferred and/or related experience with interdisciplinary teams, and care coordination.
NATURE OF SUPERVISION: - Responsible to: Clinical Manager, Emergency Psychiatric Services
ENVIRONMENT: -Bloodborne pathogen: B Multiple settings and variable unpredictable situations including inpatient, and primary care settings. May work in traditional clinic office setting and/or ED/inpatient setting. BH Clinical Navigators are based at the OPBH Clinic, part time in the Emergency Department/medical units and, when needed, mobile to meet patients in the community.
PHYSICAL REQUIREMENTS: Ability to move around settings of care. Requires close work, good vision, dexterity to write as well as to use computer keyboard. Ability to handle stressful situations.
